A Mental Disorders DBQ (Disability Benefits Questionnaire) is a standardized VA form used to evaluate mental health conditions for disability compensation purposes. The DBQ documents symptoms, diagnoses, social and occupational impairment, behavioral observations, treatment history, and functional limitations. It’s purpose is to help establish any diagnoses and to describe how symptoms are impacting your functioning, which the VA then using to establish a rating.
